Ambition Community Healthcare are a leading provider of complex healthcare services, supporting adults and children living with complex and life limiting conditions across Norfolk, Suffolk and Cambridgeshire. We have an exciting new role available within our office based team.
We are looking for a new Care Coordinator to join our team to support the Registered Manager in the daily operations of care co-ordination and office administration.
What does the role entail?

• Managing the rostering software system ensuring all packages of care are fully staffed and receive rotas in a timely manner.
• Ensuring adequate cover for holiday and sickness.
• Liaising with Support Workers daily to ensure they are aware of any changes to rotas and updating software.
• Liaising with new staff and clients to arrange meet and greets and shadow shifts.
• Liaising with the Nursing team to ensure delivery of training for packages of care in line with the clients need.
• Dealing with queries that come through to the office via telephone.
• Completing wellbeing calls to all of our staff as part of their induction process
• Conducting staff supervisions
• Assisting with on-call out of hours on a rotational basis

Job Types: Full-time, Permanent
Pay: £24,000.00-£26,500.00 per year
