What you'll be doing
The KEYS (Kids Early Years) Program is an initiative which has been developed by the Service Delivery Reform Group in Western Sydney. This program seeks to provide a more comprehensive and systemic approach to service provision and coordination for vulnerable families (with children under 5) in the area. It aims to remove organisational barriers and deliver care that is value based and person-centered. The role will assist in empowering the existing system to deliver integrated and wrap around services to vulnerable families in Western Sydney.

SCHN Specialist Keys Navigators will work within a multiagency team, to provide specialised information and assistance to key service providers – to identify vulnerable families and their needs, support access and co-ordination of quality services between agencies to families, and ensure there is a coordinated shared family plan for ongoing support to families.

In addition, the worker will be responsible for providing specialist leadership and consultation to the KEYS Navigation team and broader service system in relation to disability needs. This includes development of service system navigation pathways with key stakeholders, developing capability uplift in service providers and developing a systemic approach to building sustainability within the service sector.

The worker will play a key role in developing and participating in service development and continuous quality improvement to ensure the project meets key deliverables.

SCHN Specialist KEYS Navigators will be employed by The Children’s Hospital at Westmead but be part of an interagency team based at Wentwest, Westmead. They will maintain strong links with CHW.
  • Tertiary qualification in a relevant allied health related discipline and registration as required and demonstrated post graduate experience in the delivery of services and/or case management to vulnerable children and their families with specialist knowledge in disability
  • Demonstrated understanding of the service system which supports children and their families, and the interconnectivity between government and non-government agencies involved in supporting families.
  • Demonstrated ability to build strong and effective relationships with both internal and external stakeholders and experience in the development of interagency collaborations and innovative ways of working.
  • Demonstrated organisational and problem solving skills, ability to prioritise and meet deadlines and work independently in a dynamic and evolving program.
  • Excellent interpersonal skills and written skills, including demonstrated capacity to manage conflict and difference. Including experience in managing interagency relationships and partnerships.
  • Experience in managing effective interagency relationships and partnerships and working with individuals and services to increase capability within services.
  • Demonstrated skills in service planning, system redesign and involvement in quality improvement activities.
  • Excellent IT skills including ability to learn and adapt to new and developing systems.
