Yorkleigh Surgery is seeking a full time Care Navigator to join our friendly team.
This is a varied role which includes:

• Interacting with patients both face to face, via the telephone and digitally.
• Updating patient computerised records
• The processing of emails from healthcare professionals and pharmacies and the actioning of prescription requests and any associated queries.
• Processing internal messages in relation to patient records and healthcare.

No two days are the same! Therefore, we are seeking a team member who is proactive, enthusiastic, and friendly and who is able to multitask and work well within a team but can also work using their own initiative.

• The position is for 37.5 hours per week. Hours are worked between 07:30-18:30 on a rota basis.
