Care Coordinator

We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a crucial role in ensuring that patients receive the highest quality of care by effectively managing their social care journey. This position requires strong organisational skills and the ability to work collaboratively with various healthcare professionals, patients, and their families. The ideal candidate will have experience in an office environment and possess excellent communication skills.
Responsibilities
- Serve as the primary point of contact for service users, addressing their needs and concerns throughout their care process.
- Coordinate appointments, follow-ups, and referrals to ensure timely access to necessary services.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Collaborate with healthcare providers to develop and implement personalised care plans for service users.
- Educate service users and their families about resources, and support services available to them.
- Monitor service user progress and communicate updates to relevant stakeholders as needed.
- To allocate shifts to carers ensuring care is delivered to service users.
- To participate in on call duties during office closure on a rota basis.
- Previous experience in an office setting is preferred, demonstrating strong administrative skills.
- Excellent verbal and written communication abilities, with a focus on service user interaction.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Proficiency in using office software and electronic health record systems.
- A compassionate approach to service user care with a commitment to improving health outcomes.
- Ability to work collaboratively within a team environment while also being self-motivated.
- Full Driving Licence & Access to a Car for work purposes.
- Right to Work in the UK.
- 12 months previous care experience is desirable though not essential.
Job Types: Full-time, Permanent
Pay: From £28,000.00 per year
