Staff Nurse - Nursing Home (West)

Working hours: 5 days per week, Mon-Sun 7am-7pm shift
Responsibilities:
- Ensure nursing care services are safe, competent, and individualized.
- Maintain high standards in documentation, treatment, patient monitoring, and infection control.
- Allocate necessary equipment to meet care demands.
- Build rapport and communicate effectively with patients and their families or caregivers.
- Coordinate patient care with physicians and provide direct care.
- Assess patients’ conditions and serve as a liaison between families and healthcare providers.
- Participate in Multidisciplinary Team meetings for seamless care delivery.
- Implement safety measures for patients and report incidents to the Nurse Manager/Nurse Clinician.
- Maintain thorough documentation and accurate data compilation on referrals, inquiries, and attendance.
- Monitor, reassess, and evaluate patients’ conditions; support families coping with advanced illness.
- Perform nursing tasks such as medication administration and wound dressing as required.
- Follow up to ensure continuity of care and adherence to treatment plans.
- Registered Nurse with the Singapore Nursing Board (SNB).
- Minimum of 1 year of experience in nursing home.
- Diploma or Degree in Nursing from SNB-approved training institutions.
