Job Summary:
Start date for the role will be mid to late Jan 2025.
We are seeking a Community Physiotherapist to join our team 3 or 4 days per week. The role covers Melbourne South East Suburbs and varies day to day with approximately 6 visits per day dependant on the needs of the client or location in their homes around the South East surburbs.
First appointments generally start 8.30am and last appoinment 3.30pm.
The successful candidate will ideally have a minimum of 1 years experience working in the community setting and be familiar with Home Care Packages (HCP), & Short Term Restorative Care (STRC).
As a Community Physiotherapist, you will be responsible for providing high-quality physiotherapy services to clients in their homes or community settings.

The Role:

• As a Community Physiotherapist at Body Right Healthcare, you will provide evidence-based clinical services across various programs and funding sources (CHSP, HCP, STRC).
• Working within clients' homes via home care packages, you will assess and devise customized physiotherapy treatment plans.
• Collaborating with our multidisciplinary team is crucial for delivering holistic care solutions
• You’ll provide one-on-one physiotherapy assessments and treatments in clients’ homes with the autonomy to manage your own schedule.

Responsibilities:

• Performing thorough evaluations of clients' physical health requirements
• Creating personalized treatment plans informed by assessment results
• Delivering hands-on physiotherapy treatments to enhance clients' mobility, strength, and overall function
• Educating clients and their caregivers on exercises, self-management tactics, and techniques for preventing injuries
• Tracking client progress and modifying treatment plans as necessary
• Working alongside other healthcare professionals to ensure seamless care for clients
• Offering high-quality, evidence-based physiotherapy interventions
• Conducting detailed assessments and formulating customized treatment plans
• Collaborating within a multidisciplinary team to effectively address client needs
• Accurately evaluating client conditions and devising appropriate treatment strategies
• Working independently to prioritize tasks, keep records, and provide patient reports.

Skills:

• Ideally 1 years experience working as a Physiotherapist in the community setting
• Familiarity with Home Care Packages (HCP), Short Term Restorative Care (STRC)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Excellent clinical assessment and treatment skills

Qualifications/Certificates :

• Bachelor's degree in Physiotherapy
• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• National Police Check Certificate
• Insurance
• APA membership
