• Report to the Director of Nursing
• Complete initial and on going assessments to identify the physical psychosocial and environmental needs of nursing home patients.
• Assess patients conditions and chart observations and perform evaluations
• educate patients and their families on proper healthcare strategy and procedures
• make recommendations for devices or tools that might improve the patients quality of life
• provide effective communication to patients/ clients, their family members, team members and other healthcare professionals
• supervise staff assigned to you and utilize them to accomplish facility goals and exceeding customer service standards and providing quality healthcare.
• Complete Reports as required and participate in any investigations
• Other Duties as assigned
