Independence at Home Senior Care and Support Worker

We are recruiting for a Senior Care and Support Worker within the Independence at Home Team within Denbighshire.
The successful applicant will be expected to support the delivery of high quality person centred support from a range of staff. This includes having effective conversations with citizens to identify what matters to them and be able to design and implement support plans ensuring assessed needs are met and to promote independence, choice and wellbeing for citizens across Denbighshire.
The successful applicant will also be expected to recruit, induct, supervise, support and deploy support workers safely in the community.
Appointment subject to Disclosure & Barring Service Check, satisfactory references and registration with Social Care Wales.
