Staff Nurse - Nursing Home (West)

FULL TIME

Location: West area
Working hours: 5 days per week, Mon-Sun 7am-7pm shift
Responsibilities:
  • Ensure nursing care services are safe, competent, and individualized.
  • Maintain high standards in documentation, treatment, patient monitoring, and infection control.
  • Allocate necessary equipment to meet care demands.
  • Build rapport and communicate effectively with patients and their families or caregivers.
  • Coordinate patient care with physicians and provide direct care.
  • Assess patients’ conditions and serve as a liaison between families and healthcare providers.
  • Participate in Multidisciplinary Team meetings for seamless care delivery.
  • Implement safety measures for patients and report incidents to the Nurse Manager/Nurse Clinician.
  • Maintain thorough documentation and accurate data compilation on referrals, inquiries, and attendance.
  • Monitor, reassess, and evaluate patients’ conditions; support families coping with advanced illness.
  • Perform nursing tasks such as medication administration and wound dressing as required.
  • Follow up to ensure continuity of care and adherence to treatment plans.
Requirements:
  • Registered Nurse with the Singapore Nursing Board (SNB).
  • Minimum of 1 year of experience in nursing home.
  • Diploma or Degree in Nursing from SNB-approved training institutions.
