Responsibilities:

- Lead and supervise healthcare team including nurses and allied health professionals;
- Develop clinical guidelines , execute and monitor centre workflow to ensure daily operation including Core Centre and Service Point;
- Work with Medical Consultant of the Scheme and act as case manager to organize and coordinate care services from Family Doctors and Network Health Services professionals as appropriate;
- Coordinate Nurse Clinic for client clinical care including conducting complication screening, coaching and chronic disease management;
- Participate in strategic planning and promotional activities including supervision of subordinate in outreach planning and activities;
- Developing primary healthcare education resource and training for healthcare team where appropriate;
- Can adopt flexible working hours with Evening, Saturday or Sunday duties on occasions;
- May be required to rotate among various centres in Kwun Tong District.
- Degree in Nursing and registered with the Nursing Council of Hong Kong;
- Over 5 years experiences at the supervisory level in community healthcare or over 10 years experiences at primary healthcare services;
- Recognized post registration training in Primary Health care Nursing / Community Nursing / Diabetes care / Cardiology / Gerontology would be an advantage
